Health History Form
The Health History form (Section 5 in the PIAA packet) should be completed, taken to the physical appointment, and then submitted with the PIAA Physical form. FamilyID for the 2023 Fall season opens on Thursday, June 1, 2023. The registration period will initially close at 11:59pm on Sunday, July 30, 2023.

Physicals on other forms will not be approved. - Signed by a doctor/physician - Current, dated on or after 6/1/23 - Uploaded to the student profile on FamilyID (paper copies will not be accepted)
